Two things, both easy
- Exchange transaction export (CSV) from every account, covering the full history. You get exchange-specific instructions for finding the export along with the diagnosis.
- One public wallet address per chain — preferably the one you withdrew to from the exchange. We don't need a wallet file, and we never need seed words or passwords.
- If you can't remember a single address, that's not a problem: they can be found from the transaction IDs on the withdrawal rows.

The diagnosis is work, not a form. That's why it tells you something you can't guess yourself, and why we only do a limited number at a time. The breakdown and calculation are the work you pay for in the reconstruction — the diagnosis tells you the size of the mess, the reconstruction its contents. If the numbers show you don't need us, we'll say so plainly and charge nothing.

Quick estimate in six questions
This estimate is based on what you remember — not on your data. It gives a price range and service level, but not what your history actually contains. The real transaction count is usually higher than the guess. That's what the diagnosis is for.
